WHAT SACRAMENTAL CERTIFICATES DO WE NEED?
You will need to obtain copies of your baptismal and confirmation certificates from the church(s) where you first received these sacraments. The certificates may not be dated more than six months prior to your wedding date. Make copies of the documents before submitting them to Old St. Pat’s as they will not be returned to you.
If your baptism took place outside of the United States, furnish a stamped addressed envelope to the church for your baptism certificate and certified letter of translation.

WHAT IF I’M MARRYING SOMEONE FROM A DIFFERENT FAITH?
If the person is of another Christian faith, bring a copy of their baptismal record. If the person is not baptized, your priest will advise you about completing the Chancery Office paperwork requirements.
TELL ME ABOUT THE WEDDING AFFIDAVITS.
Witness forms to establish freedom to marry are required.
Parents are the preferred witnesses, but close relatives or friends of long standing also can be witnesses.This is a total of four witnesses. Your witnesses must sign the affidavits in front of their own priest, deacon or clergy and have them stamped with the church’s seal and sent directly to Old St. Patrick’s Church.

WHEN SHOULD WE GET OUR MARRIAGE LICENSE?
You must obtain your marriage license from the Office of the Clerk of Cook County in person no earlier than 60 days before your wedding date, and no later than one day prior.
After you are married, the priest will sign your marriage license. Old St. Pat’s will send it to the Clerk of Cook County’s office. You can obtain a certified copy of the marriage license by contacting the Clerk of Cook County’s office 15 business days after your wedding.
A license obtained in Cook County cannot be used in any other county or state.

In any case, the license should be at Old St. Pat’s no later than 3 days prior to the wedding. Please consult our Wedding Coordinator if there are any questions regarding the documents.
